Business Card Request Form

As an official member of the IAAP Los Angeles-Metropolis Chapter, you will receive business cards by submitting the form below.  Please enter your information as indicated.  Once you have completed the form, please submit it.  If you select delivery, we will send your cards to you via U.S. Mail to the address you indicate.  For those who select pick up, the cards will be prepared for pick up at the next upcoming meeting.  Card orders submitted 48 hours or less before a  monthly meeting may not be ready for pick up.  Thank you for your cooperation and participation in IAAP Los Angeles-Metropolis Chapter.
